  • How do I pay for my PT care?

    We accept all credit and debit cards, Venmo, and payments from your flexible spending account (FSA) or health savings account(HSA) through your insurance. We also offer bundle treatment plans in the case that you desire routine visits throughout the year or in the unfortunate case that you have a major injury and will require a large number of PT visits.

  • Rogue Rehabilitation and Performance is considered an out-of-network provider for insurances not listed above. More specifically, we do not accept an insurance assignment of benefits. As a cash-based physical therapy clinic, we still diagnose, document, code, bill and collect similarly to any traditional PT you may have seen in the past. The caveat is, the physical therapist and the patient are not bound by the constraints of insurance, which can determine where you receive treatment, the number of visits you are allowed, and can influence what treatments you are provided. We at Rogue Rehabilitation and Performance know this approach is against the grain, but we can charge less because the simplified cash-based fee structure streamlines billing and does not require hiring billing personnel or paying fees to a third party billing service

    Can I submit a paid claim to insurance?

    Yes, Insurance companies reimburse patients according to their own fee schedules, and each insurance company has a different rate of reimbursement for different codes. Although it varies with insurance plans, most patients receive some level of reimbursement(40-80%) or their payment is applied to their deductible. The providers at Rogue Rehabilitation have complete control over the codes submitted and billed. Our focus is to make your treatment plan as efficient and financially friendly as possible!

    You will receive a claim/bill during each visit which includes all of the required information. This claim can be submitted to your insurance company in order to be reimbursed or the amount applied to your deductible?

  • You are not required to get a referral to get physical therapy services in Utah. We are open to all customers who are seeking physical therapy.

  • Rogue Rehab accepts both insurance covered and non-covered patients seeking physical therapy. Depending on your coverage, you may be eligible for insurance based therapy. See our payments site for more information about what payment plan works best for you.

  • We do not set any standards or restrictions for physical therapy because rehabilitation should be customized for you personal treatment.

    EX: While cupping therapy may be the most beneficial for one patient, you may see the best results through dry needle therapy.
